The Community Health Advocate position is located in Indianapolis, Indiana.

Job Duties

The Community Health Advocate is responsible for providing outreach, social support, care transition, and community resource information to patient/clients and their families.

The Community Health Advocate promotes access to medical and social services assisting clients in utilizing available resources; including scheduling appointments, accompanying patients to appointments as needed, educating patients about available community resources, referring patients to local service agencies, helping complete forms and applications for services, coaching patients on self-management, conducting home and or community visits, and communicating client related information back to the clinical team.

Experience

6 months experience working in a Community Health Worker (CHW) role or in another position with the provision of social services, supportive services, personal care, or related duties. Bilingual in Spanish-English preferred.

Education

High School education or GED required.

License

Valid Indiana driver’s license and insurance required.

The Patient Service Representative position is located in Indianapolis, Indiana.

Job Duties
* The Patient Service Representative works in conjunction with providers and clinical staff to take care of patients’ administrative needs.
* This position is vital to giving the patient the “Patient’s First” service and treatment they deserve as soon as they walk into our facility.
* Primary responsibilities include checking in patients for appointments, collecting patient payments, answering telephones, taking accurate messages, scheduling appointments, and checking insurance eligibility.
* This position utilizes our electronic medical record system to verify and update patient demographic information, as well as, posting and balancing of payments collected.

Experience

* One year of experience in a healthcare environment or a minimum of two years of experience in a customer service focused role.
