Job Description, Responsibilities, Standards and Qualifications
The Forensic Community Coordinator will serve as a liaison between the consumer, the court, the community, and the hospital. Essential components include:
Identify viable community behavioral health treatment options for State and Superior Courts seeking treatment as alternatives to incarceration:
Develop and maintain a working relationship with other agencies in the community to stay current about available resources
Serve as liaison between courts and community programs to facilitate service acquisition for individuals
Provide case management and educational sessions focused on competency remediation for individuals ordered by the State, Superior or Juvenile Court to receive these services in the community:
Provide educational training to individuals mandated to outpatient competency remediation services.
Alert forensic psychologist assigned to the case of competency status.
Provide documentation of sessions and other information to evaluating forensic psychologist.
Coordinate with Forensic State Office to ensure that the statewide community restoration tracking system is updated.
Facilitate referrals to other behavioral health treatment programs if needed.
Provide case management and oversight of individuals who have been adjudicated Not Guilty by Reason of Insanity (NGRI) and conditionally released or have been adjudicated Not Competent to Stand Trial and Not Restorable (IST-NR) and released on civil outpatient commitment / bond with mental health monitoring.
Regular contact with individuals to monitor clinical status and assess progress in community integration.
Monthly contact with community agencies to verify attendance and participation and ascertain status.
Document of all client contacts and community agency contacts.
Provide monthly reports to the respective Hospital / Community Forensic Director(s) regarding an individual’s progress in relation to their Recovery Plan.
Provide to Forensic Program Associate at the hospital(s) all monthly contact notes for accurate entry into AVATAR.
Track and schedule dates for Court updates. Coordinate with forensic psychologist to complete status update reports for the Court.
Attend court hearings, as needed.
Participate at hospital recovery team meetings as required.

Qualifications:
Master’s degree in a related field from an accredited college or university AND Two years of experience performing work related to area of assignment OR Bachelor’s degree in a related field from an accredited college or university AND Five years of experience performing work related to area of assignment OR One year of experience at the lower level Healthcare Prgm Consultant 2 (HCP091) or position equivalent. Note: Some positions may require licensure.
